Stages Of Pressure Sores

Pressure Risk Categories

1	Category I – Non-Blanchable Erythema	Defined as a non-blanchable erythema of intact skin. Discolouration of the skin, warmth, oedema, induration or hardness may also be used as indicators, particularly in people with darker pigmentation.
2	Category II – Partial Thickness Skin Loss	Defined as a non-blanchable erythema of intact skin. Discolouration of the skin, warmth, oedema, induration or hardness may also be used as indicators, particularly in people with darker pigmentation.
3	Category III – Full Thickness Skin Loss	Defined as being of full thickness skin loss, where the subcutaneous fat may be visible to the naked eye, but the skin damage has not exposed bone, tendon or muscle. Slough may be present but does not obscure the depth of tissue loss and may include undermining and tunnelling.
4	Category IV – Full Thickness Tissue Loss	Defines and characterised by a deep wound that reaches the muscles, ligaments, or even bone. They often cause residents to suffer extreme pain, infection, invasive surgeries, or even death.
